Two forms of identification are required.
- One form of identification must be photo identification – such as a driver’s license, DMV ID card or a passport.
- The other form of identification can be a registration card, military card, voter registration card, employer identification card, or school identification.
COST – There is a $10.00 fee for the first card and $5 for each additional card.
